This is available from the AQIS web-site. Murdoch’s importer No is: 99501213 The form requests details of: The work must be carried out within a Quarantine Approved Facility at Murdoch University. Send the form, together with payment to: Australian Quarantine and Inspection Service You must send a copy of the form to the IBC Secretary, Human Resources If you are not sure about whether you need an AQIS permit, see the AQIS web site. In general anything likely to contain biological material (soil water, plant or animal tissue or objects derived therefrom) needs a permit. Information on fees and charges can be found on the AQIS website AQIS will usually make a decision within several days. If the request is approved they will issue a licence. The person you are importing from will need to include the licence number in the package, otherwise customs will stop the goods at the point of entry and hold until you supply the licence number.
