Application         - for Human Research Ethics Approval

Amendment         - to make changes to an approved project

Annual Report     - required annually on the anniversary of outright approval

Closure                - to close the file once all contact with participants is completed

Extending            - to extend the approval period beyond the initial 3 years

Adverse Event    - If something goes wrong, or if something unexpected occurs, use this form to report it

If you need an 'unlocked' version of any of the forms, please email human.ethics@murdoch.edu.au

Always download a fresh application form. Do not save a blank form to use later. The forms are regularly reviewed and updated. Out of date forms are not accepted by the HREC.

Complete the form

Obtain Advice if needed. Consult your supervisor, the Advice and FAQ section of the web-site or the Research Ethics Office.

Submit the completed form to the Research Ethics Office.

Applications can be submitted at any time. Low-risk applications will be expedited, other applications will be held over to the next HREC meeting. Applicants will be advised whether or not an application is being expedited.
